Expect HIV Treatment Interruption for Thousands if Congress

Doesn’t Restore $6.6 Billion Shortfall

Washington, DC: Today, President Obama released the details of his 2010 budget for Global Health, and outlined a new six-year global health initiative. However, the only aspect of the plan that appears “new” is a dramatic decrease in funding for programs to address HIV, malaria and tuberculosis.

Last year, with the help of then-Senators Biden and Obama, Congress reauthorized the US global AIDS, TB, and malaria programs (commonly called PEPFAR) at $48 billion over 5 years. A modest estimate to extend that level of funding over six years to match the new initiative would be $57.6 billion. The President’s current proposal only calls for $51 billion for the same programs – a $6.6 billion cut. This means there will be dramatic reductions to funding for AIDS, TB, malaria, and still very little left over for vital expenditures like maternal and child health and health system strengthening.

The President had previously committed to do significantly more on global AIDS, TB and malaria. In his statement, President Obama highlighted his commitment to the Lantos-Hyde US Global Leadership Against HIV/AIDS, TB and Malaria Act while he was a Senator. This legislation authorized $48 billion over five years for AIDS, TB and malaria. During his campaign, at several junctures, he promised to fully fund PEPFAR, including a written pledge to support $50 billion over five years for global AIDS alone. President Obama also committed to the training of one million new health workers globally.

“President Obama repeatedly committed to ensuring the US does its fair share to fight AIDS around the world. But this budget’s drastic cuts to funding for AIDS, TB and malaria shows that his promises were just rhetoric,” said Kaytee Riek, Director of Organizing for Health GAP. “Effective programs, and the people whose lives are saved by those programs, will suffer.”

The Global Fund to Fight AIDS, TB, and Malaria is in particular danger. “The Global Fund board meeting is taking place right now and we are going to be making some difficult decisions about how to respond to the $5 billion funding shortfall. We had hoped that President Obama would fulfill his campaign promise with this budget and contribute the U.S. fair share to this lifesaving multilateral program.

Sadly, with a mere $366 million increase in US funding for all AIDS, TB and malaria programs in 2010, there is not enough room to fully fund the Global Fund, and means the Global Fund may have to start cutting existing and future grants,” said Asia Russell, Director of International Advocacy for Health GAP and a Board Member of the Global Fund.

The budget now goes to Congress for debate and approval. “This budget does not contain the funding for the Administration to meet targets Congress required when it reauthorized PEPFAR. In addition, this budget does not go nearly far enough on other global health priorities, including maternal and child health and health systems strengthening”, said Siham Elhamoumi, a Health GAP core member and leader in the Vermont Global Health Coalition. “Fortunately, Senator Leahy, Representative Lowey, and others in Congress can ensure that the Administration has the resources they need to save millions of lives worldwide. We are hoping they will act.”

Since taking office, President Obama’s budgets have shifted funding away from AIDS programs, and his commitments to fighting AIDS have not even kept pace with inflation, let alone increased to the level he promised. As has been highlighted by recent stories in the Boston Globe and Newsweek, a flat-funded budget means that people who were tested under US-sponsored programs and promised treatment when they got sick are now being turned away.

Clinics receiving money from the US government are being instructed to enroll no new patients in life-saving treatment programs. Doctors are being
instructed to ration drugs and face choices between giving their dwindling supplies to young mothers or children. People who are turned away often have nowhere else to go, and will die.

“Obama’s broken promise is killing people with AIDS around the world. We cannot backtrack on our commitment to saving lives,” said ACT UP Philadelphia member Jose de Marco.

The AIDS epidemic among blacks in some parts of the United States is as severe as in parts of Africa, according to a recent report published by the Black AIDS Institute, an HIV/AIDS think tank focused exclusively on black Americans. “AIDS epidemic is not over in America, especially not in black America,” says the report. “AIDS in America today is a black disease,” says Phill Wilson, founder of the Black AIDS Institute. Although black people represent only about one in eight Americans, one in every two people living with HIV in the United States is black, the report notes.

The report uses just-released data from UNAIDS and existing CDC and Census data to highlight grim statistics:
AIDS remains the leading cause of death among black women between ages 25 and 34. It’s the second-leading cause of death in black men 35-44.
In Washington, more than 80 percent of HIV cases are among black people, that’s one in 20 residents. “Five percent of the entire population (in DC) is infected…that’s comparable to countries like Uganda or South Africa,” Dr. Anthony Fauci, director of the National Institute of Allergy and Infectious Diseases, told CNN for a recent documentary.
According to this report, if black Americans made up their own country, it would rank above Ethiopia (420,000 to 1,300,000) and below Ivory Coast (750,000) in HIV population. Both Ethiopia and the Ivory Coast are among the 15 nations receiving funds from the President’s Emergency Plan For Aids Relief. The United States has given about $15 billion to PEPFAR nations in the past five years.

The Black AIDS Institute says it’s not criticizing the federal government for helping poorer countries cope with the AIDS epidemic. Rather, it’s saying the “AIDS epidemic in the U.S. is not getting the kind attention that it merits.” “We understand the needs of black folk in Johannesburg (South Africa),” Wilson says. “Why can’t we understand the needs of them in Jackson, Mississippi? We understand the needs in Nigeria or Botswana, why not understand the needs of Los Angeles or Oakland?” Wilson says more needs to be done to prevent the spread of HIV in this country. The report states that the U.S. government “increased spending on HIV prevention, treatment and support programs for low-income countries dramatically, at the same time that domestic remained all but flat.” According to the Kaiser Family Foundation, domestic prevention efforts make up the smallest part of the HIV/AIDS budget, the 2009 budget request includes $892 million for domestic HIV prevention efforts, the same as in 2008. Obama and his cronies have NOT compiled and/or released a 2010 budget for The United States.

In this report, Wilson and others urge the federal government and private foundations to significantly increase funding for HIV prevention and treatment programs. The report also calls on international agencies to hold the U.S. government accountable, whatever that means, for failure to address the HIV/AIDS epidemic in its own country (despite lauding it for its PEPFAR efforts). It also urges black communities in the United States to fight the stigma and overcome prejudice associated with being infected with HIV. Prejudice? Maybe the black community should practice some self-control and a little personal responsibility!!! Maybe the black community should stop blaming outside forces and the lack of tax-payer money for their problems!!! Maybe the black community should tell Bitch Obama not to lift the ban on people with HIV traveling to the U.S.

The U.S. Department of Health and Human Services, in 1987, added HIV/AIDS to the list of communicable diseases that disqualified a person from entering the United States. Dumb-fuck Obama says that in overturning the ban, he is finishing a process begun by George W. Bush. DES MOINES, Iowa – A blank document from the Iowa Department of Public Health has been discovered online, designed to be filled in with the name of an H1N1 virus victim who is required to relocate from his or her home to a quarantine facility.

The form, which began appearing today in e-mails and on the Internet, has concerned a confused public already swimming in conflicting reports about the severity of the swine flu and intrusive government measures that many fear may be taken if the disease becomes a pandemic.

The Iowa document, which WND confirmed with state officials is authentic, has done little to calm the public’s fears.

“The Iowa Department of Public Health has determined that you have had contact with a person with Novel Influenza A H1N1,” the form reads. “The Department has determined that it is necessary to quarantine your movement to a specific facility to prevent further spread of this disease.

“The Department has determined that quarantine in your home and other less restrictive alternatives are not acceptable,” the document continues, before listing mandatory provisions of compliance with relocation to a quarantine facility.

The blank-form document, which has no name or case number listed, is titled a “Facility Quarantine Order,” and though Iowa Department of Public Health Medical Director Patricia Quinlisk confirmed the state has the form, she told WND it’s highly unlikely it will ever be used.

“We’ve had these kinds of template orders for years, but we hardly ever use them,” Quinslick said. “I can count on two hands the number of times – in 20 years – that we’ve had to relocate a person because of quarantine.”

Quinslick wasn’t certain who released the blank document, or who signed her name to the bottom of some versions circulating on the Internet, but she wasn’t concerned that it’s been made public.

“We’re not trying to hide anything,” she said. “This is the kind of form we use.”

Quinslick told WND her department has prepared similar documents for several contagious diseases to “have them ready just in case.”

But at the same time, she said, state law requires that when IPDH does need to quarantine someone, it be done in the least restrictive way possible, which typically means in the person’s home.

“Usually when we relocate, it’s only because they have no place to be, like a homeless tuberculosis patient,” Quinslick said, “One time, I recall, we relocated a homeless man to his relative’s house.”

The Facility Quarantine Order for the H1N1 virus that has been circulated online illustrates Quinslick’s point.

Following the order that a person be removed from his or her home, the form contains a blank for explaining the reason, including one of the following: “the person violated a previously issued home quarantine order, the person does not have an appropriate home setting conducive to home quarantine, etc.”

Quinslick told WND it was doubly unlikely the state would use the form, since the swine flu has proven less dangerous than earlier estimates.

“I don’t anticipate using [the form] for H1N1, since it’s been shown to be a very mild disease,” Quinslick said. “We found this spring it’s not as serious as feared.”

As for fears that Iowa is preparing detention centers for rounding up the people who refuse swine flu shots, Quinslick was quick to dismiss the idea.

“Iowa has no relocation facility currently,” she said. “If we can’t quarantine a person in their home, we usually use a hotel room. There’s not a facility, no gymnasium or anything like that set up.

“And I’ve never been in on any discussion of forcing people to be vaccinated,” she added. “If there’s a highly contagious outbreak and a person refuses vaccination, that’s fine, they don’t need to be vaccinated, but they may need to stay home until we’re certain the danger has passed.”
